Moxie’s Pet Service Inc fees and packages are non-refundable and non-transferable. If I choose to reserve specific dates for any kind of service I am liable to pay for those services. I understand if I reschedule or cancel any appointment within a 48 hour period I will be charged 50% of the original service charge. If on a holiday week I will be charged for the entire cost of the rescheduled or canceled service. For some holidays there is a $5-10 per visit holiday charge the week or day of major holidays. I understand the rules above apply to any pet(s) attending any services and all special events. I understand any invoice not paid within a 30 day period from the date of invoice issuance will result in a late fee of $50 per 30 days past the generated invoice date. Invoices not paid within 90 days past the due date will be assigned to a collection agency, including all accrued late fees and costs associated with the collection process. I certify that I am eighteen years of age or older and have the legal capacity to enter into a binding contract. Further, I certify that I have read and understand the rules and regulations set forth in this agreement. I agree to abide by the rules and regulations and accept all the terms, conditions and statements of this agreement.
